Malibu camp Climbing Wall Liability Release

Dear Athlete and parents,

This summer we will be offering our athletes as an optional activity use of the Runner's Workshop Malibu / Cottontail Ranch climbing wall. The climbing wall is an excellent team and individual self-confidence building activity. There will be a one-time charge of $10.00 for anyone interested in using this activity. (The one time fee covers unlimited use during free time during the first two days at camp).

To be able to take part in the climbing wall you will need a parent signature on the form below and $10.00. You may either mail this and a $10.00 check at least one week prior to camp or bring it to camp with you. No athletes will be allowed to climb without the permission slip and fee.

Please read the following liability relaase agreement for the climbing wall at Runners Workshop Inc. and Outdoor Learning Adventures before signing:

ACTIVITY RISK
As a guest at Runners Workshop Inc. and Outdoor Learning Adventures, I, the undersigned, recognize that Runners Workshop Inc. and Outdoor Learning Adventures are located in a rustic setting with natural and artificial hazards (including surface and subsurface conditions).

CLIMBING WALL RESPONSIBILITY:
The undersigned will be given basic climbing wall instruction prior to use of the wall, yet there is potential for the participant to act or fail to act in a manner that could contribute to injury, loss, or death. The participant's safety largely depends upon his/her ability to carry out simple instructions and follow any instructions given by the instructors.

WAIVER OF LIABILITY
In order that my minor child may participate in the activity or activities listed above, I forever waive my right (and my child's right) to sue Runner's Workshop Inc. and Outdoor Learning Adventures (including directors, staff, employees and other contracted parties) for any injury my minor child may suffer arising our of my minor child's participation in this activity. I understand that by signing this document all liability of the Runner's Workshop Inc. and Outdoor Learning Adventures (including its directors, staff, employees, and other contracted parties) to myself and my minor child for any injuries my minor child may suffer arising out of my minor child's participation in the activity or activities listed above will be forever extinguished.

SIGNER STATEMENT OF AWARENESS
I/We, the undersigned, have read and do understand and agree to the foregoing agreement, warnings, waiver, and the assumption of risk. We assume the risk of injury from the above danger, and waive liability from injury, therefore from Runners Workshop Inc. and Outdoor Learning Adventures and its staff and volunteers.
