Camp Physical
Each runner must have a physical within 12 months prior to attending camp.
You can use either your school’s physical form or ours. Click below to access ours. Please have the doctor complete sign the form and either scan and email to runnersworkshop@mac.com or mail to us (not the camp) at Runner’s Workshop, P.O. Box 5028, Los Alamitos, Ca. 90720.
